	PROFESSIONAL TIN Number Birth Certificate PRC License ID. Marriage Certificate (if married)	PROFESSIONAL Notarized Lease Contract from Lessor (if using an office address)
One Person Corporation (OPC)	TIN Numbers Any Valid Government Issued ID List of Employees/ Affidavit of no Employee	A business address located in a commercial district is required.
